Short answer

If dandruff is not treated, flaking and scalp irritation may continue, become more noticeable, and sometimes reflect seborrheic dermatitis.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What untreated dandruff can mean

Dandruff commonly causes loose white or yellowish flakes, itching, and a scalp that may feel dry or oily. Leaving it alone does not usually cause serious harm, but the cycle can persist: scratching irritates the skin, and irritated skin can feel itchier. Persistent scale may be part of seborrheic dermatitis rather than simple dandruff. Review Dandruff and Seborrheic Dermatitis for the difference.

Scratching can leave small breaks in the skin and may make the scalp tender. Flakes can collect on hair, clothing, or bedding, which can be frustrating even when the condition is medically mild.

What you can do

Use a gentle scalp routine and avoid picking, vigorous scratching, or very hot water. A clinician or pharmacist can help select a medicated shampoo suited to your symptoms; Medicated Shampoo Treatment is one option, while Topical Antifungal Treatment may be considered when yeast-related inflammation is suspected. Follow the product directions, allow treatment time on the scalp when instructed, and stop if you develop marked irritation.

Track whether itch, oiliness, and flaking improve, and note any new redness or sore areas. Helpful symptom pages include Itchy Scalp, Oily Scalp, and Flaky Scalp.

When to seek care

Arrange a dermatology assessment if the scalp is painful, very red, swollen, crusted, bleeding, or losing hair in patches, or if careful self-care does not settle the problem. Seek prompt care for spreading inflammation, pus, or fever. These features can point to another scalp disorder or infection rather than routine dandruff.

Questions for your doctor

Could this be seborrheic dermatitis or another scalp condition? Which shampoo or leave-on treatment fits my scalp? How long should I try it, and what changes mean I should return?

Questions people ask

Can untreated dandruff cause hair loss?

Repeated scratching can break or irritate the scalp and may cause temporary shedding, but persistent or patchy hair loss needs assessment for another cause.

When should I see a dermatologist?

Seek assessment for pain, marked redness, crusting, bleeding, patchy hair loss, fever, or symptoms that persist despite suitable care.
