What happens if cancer of unknown primary is not treated?

Cancer of unknown primary means cancer has been confirmed, but tests have not identified where it began. The known deposits can still enlarge, invade nearby structures, or affect organs such as the liver, lung, bone, or brain. Symptoms depend on where the cancer is found and may include a new lump or swelling, a neck lump, unexplained weight loss, pain, breathlessness, bowel changes, or neurological problems. Not finding the primary site does not mean the disease is harmless, and a symptom does not by itself reveal its cause.

A cancer team may review pathology, perform focused imaging, and use a PET-CT scan to look for additional disease. Genomic tumour profiling may provide clues about tumour behaviour or possible treatment targets. Once the available information is assembled, treatment may include chemotherapy or another systemic approach, a procedure for a specific area, or symptom-focused care. The plan is tailored to the cancer pattern, the organs involved, general health, and personal goals.

Seek emergency help for severe breathing difficulty, new weakness, confusion, a seizure, uncontrolled bleeding, or sudden severe pain. Contact the cancer team promptly for a rapidly enlarging lump, persistent vomiting, inability to eat or drink, worsening jaundice, or a marked decline in function. Timely review can identify complications and adjust symptom support.

Ask which sites have been examined, what the pathology suggests, whether further imaging or genomic testing could change treatment, and what symptoms indicate an urgent complication. Ask how the team will review the diagnosis if new information appears.
