Shaking may start in the hands but can feel like whole-body trembling. Other early signs include clammy skin, hunger, tingling around the lips, dizziness, weakness, irritability, anxiety, blurred vision, or a pounding heart. As glucose falls further, thinking and coordination can become difficult, and confusion or unusual behaviour may develop. The same sensation can have other causes, so checking glucose when possible is useful, particularly if you use insulin or another glucose-lowering medicine.

If you are awake and able to swallow, follow your diabetes care plan and take a fast-acting source of glucose, then recheck according to that plan. Once recovered, have a meal or snack if your next meal is not soon. Do not drive while symptoms continue. If episodes repeat, record timing, meals, activity, alcohol, medicines, and glucose readings and share them with your clinician.

Call emergency services if the person is confused, having a seizure, unconscious, unable to swallow safely, or not improving after the agreed treatment. Never give food or drink to someone who is drowsy or unconscious. Repeated episodes, overnight symptoms, or a medication mismatch need prompt medical review.

Ask how to recognise and treat your episodes, when to use prescribed rescue medicine, and whether your diabetes medicines need review. Bring glucose readings and explain the relationship to meals, exercise, illness, alcohol, and medication timing.
