What do irregular periods feel like with hypothalamic amenorrhoea?

Short answer
Irregular Periods is a common presentation of Hypothalamic Amenorrhoea, and periods may become widely spaced, very light or absent.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical pattern
- Widely spaced, light or absent periods
- Possible triggers
- Low energy availability, intense exercise, stress or illness
- Review
- Pregnancy and other causes need assessment
Why the pattern can change
The hypothalamus helps coordinate signals between the brain and reproductive system. Low energy availability, intense exercise, emotional stress, illness or significant weight change can disturb those signals. Ovulation may become infrequent, so bleeding can be delayed, shorter, lighter, or missing for several cycles.
You may also notice reduced vaginal lubrication, lower sexual desire or symptoms linked with low oestrogen. These features are not specific, and pregnancy and other hormone conditions must be considered.
What to do next
Arrange an appointment if your periods become persistently irregular or stop. Share changes in eating, exercise, sleep, stress, weight, long-term illness and medicines. A clinician may check for pregnancy and other causes with an examination and blood tests, then discuss restoring adequate nutrition, reducing excessive training and supporting mental health where relevant.
Avoid using hormonal bleeding to mask the pattern without addressing the cause. Ask how bone health, fertility and hormone levels should be monitored while cycles are recovering.
When to seek care
Seek urgent care for fainting, severe weakness, chest pain, severe abdominal pain or bleeding during pregnancy. Request prompt review if periods have stopped, if eating feels unsafe or difficult, or if exercise is accompanied by dizziness or collapse.
Questions for your doctor
Could energy intake, exercise, stress or another illness be affecting my cycle? Which tests will rule out pregnancy and other hormone causes? How should we protect bone health and plan for pregnancy if that is a goal?
