What Causes Shoulder Instability?

Short answer

Shoulder instability is commonly caused by a previous dislocation or injury that stretches or tears the tissues supporting the shoulder joint.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Main symptom
A slipping, shifting or giving-way sensation
Common contributor
Previous shoulder dislocation or injury

Why the shoulder can feel loose

The shoulder has a wide range of movement, so it depends on the labrum, joint capsule, ligaments, rotator-cuff muscles and coordinated shoulder-blade movement for control. A dislocation or partial slip can stretch these structures; after that event, the joint may feel as if it shifts, catches or might come out again. Some people have naturally flexible connective tissues or repeated overhead demands that allow instability to develop without one clear injury. Weakness, poor movement control or a previous fracture can also affect stability.

What to do next

Stop movements that repeatedly produce slipping, sharp pain or a sense of giving way, but keep the arm moving gently within a comfortable range unless it has just been dislocated. Assessment may include shoulder strength, range of motion, joint control and tests for associated injury. Physiotherapy often targets the rotator cuff and shoulder-blade muscles, with a gradual return to lifting or sport. Recurrent episodes may require an orthopaedic opinion.

When to seek urgent care

Seek emergency help if the shoulder looks out of place, cannot be moved normally after an injury, or is accompanied by severe pain, numbness, a pale or cold hand, or marked weakness. Arrange prompt assessment after a first dislocation, repeated slipping episodes or a new loss of strength.

Questions for your doctor

Was there a dislocation, partial slip or soft-tissue injury? Which movements should I avoid temporarily? What strengthening plan suits my shoulder? Do recurrent episodes make imaging or specialist review appropriate?

Questions people ask

Can shoulder instability happen without a dislocation?

Yes. General joint flexibility, repeated overhead activity or reduced muscle control can contribute to instability without one dramatic event.

Can physiotherapy help?

A tailored programme can improve rotator-cuff strength, shoulder-blade control and confidence during movement, depending on the cause.
