What causes restless legs syndrome?

Short answer
Restless legs syndrome is a nervous-system movement condition marked by an urge to move the legs and unpleasant sensations during rest, especially later in the day. Iron deficiency anaemia is a very common associated condition, and chronic kidney disease is another recognised contributor. Some people have a family tendency or develop symptoms without an identifiable medical cause. Medicines, pregnancy and sleep disruption can also influence symptoms.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Core pattern
- Worse at rest, relieved for a time by movement
- Common associations
- Low iron and chronic kidney disease
What the symptoms suggest
The characteristic pattern is discomfort that begins or worsens while sitting or lying down, improves with movement, and returns after becoming still. It differs from a muscle cramp, which tightens a muscle, and from neuropathy, which may cause persistent burning or numbness. Clinicians ask about sleep, medicines, family history, pregnancy and diet, then may check iron status and kidney function.
How it is assessed and managed
Write down the time of day, the sensations, sleep disruption and anything that relieves them. Gentle stretching, regular activity and a consistent sleep routine can reduce triggers for some people. Review medicines with a clinician rather than stopping them yourself. If testing shows low iron, the cause of the deficiency should be explored before iron supplementation is chosen.
When to seek care
Arrange an appointment when symptoms are frequent, impair sleep or interfere with daily activities. Seek prompt care for sudden weakness, a swollen painful leg, chest pain or breathlessness because these symptoms need a different assessment. Ask for earlier advice if symptoms start during pregnancy, follow a medicine change, or occur with marked fatigue or signs of bleeding.
Questions for your doctor
Do my symptoms fit restless legs syndrome? Should we check iron levels and kidney function? Could any medicine, pregnancy or another condition be contributing?

Questions people ask
Does restless legs syndrome always have a clear cause?
No. It may be associated with another condition, but some people have no identifiable trigger after assessment.
Why check iron levels?
Low iron is a common association and can affect symptoms, so testing helps decide whether replacement is appropriate and prompts a search for the reason it is low.
