What causes cervical myelopathy?

Cervical spondylosis can produce disc bulges, bony overgrowth, and thickened ligaments. Together, these changes may reduce the space available for the spinal cord. A naturally narrow canal, a disc prolapse, or a previous neck injury can make compression more likely.

Because the spinal cord carries signals to the arms and legs, symptoms can extend beyond neck pain. Hand clumsiness, dropping objects, altered balance, stiff legs, or changes in walking may reflect cord involvement rather than a problem in one arm nerve alone.

Arrange a clinical assessment if hand coordination, balance, walking, or limb strength is changing. The assessment may include reflexes, sensation, grip, gait, and neck movement. Imaging helps show whether the cord is narrowed or compressed; treatment depends on the examination, imaging, symptoms, and how they are changing.

Seek urgent care for rapidly increasing weakness, new difficulty walking, repeated falls, or a new loss of bladder or bowel control. After a neck injury, worsening arm or leg weakness or numbness also warrants immediate assessment. Do not force neck stretches or manipulation while a cord problem is being evaluated.

Ask whether the symptoms suggest spinal cord involvement, where the narrowing is located, what the scan shows, and which activities should be paused. Clarify the warning signs that mean you should seek same-day care.
