What causes cervical insufficiency?

Understanding cervical insufficiency
The cervix normally stays closed and supports the pregnancy. With cervical insufficiency, it may soften, shorten, or open before labour, sometimes with little pain or few warning symptoms. A history of cervical procedures, including loop excision of the cervix, can affect cervical tissue. Previous cervical trauma, a uterine or cervical structural difference, or a prior pregnancy loss in the middle of pregnancy may also be relevant.
Sometimes no clear cause is found. The condition is assessed from pregnancy history, examination, and ultrasound measurements rather than from one symptom alone. It is different from ordinary pelvic pressure or harmless changes in pregnancy.
What to do during pregnancy
Tell your maternity team about any previous cervical procedure, late miscarriage, early birth, or cervical surgery before or early in pregnancy. They may recommend monitoring the cervix with transvaginal ultrasound and discuss options based on your history and findings.
Possible management can include observation, medicines in selected situations, or a cervical stitch. The plan is individual and depends on gestation, examination, ultrasound findings, and previous pregnancy history. Do not start bed rest or restrict activity without specific medical advice.
When to seek care
Contact your maternity unit promptly for pelvic pressure, new backache, watery discharge, bleeding, regular tightening, or a feeling that something is pressing downward. Urgent assessment is needed for fluid leakage, heavy bleeding, painful contractions, fever, or reduced fetal movement.
