What are the treatment options for scalp ringworm?

Scalp ringworm, also called tinea capitis, is a fungal infection involving the scalp and hair. A cream used only on the surface generally cannot reach the infected hair shaft, so a clinician commonly prescribes an oral antifungal medicine. The choice depends on the suspected fungus, age, medical history and other medicines.

Medicated shampoo can reduce fungal material on the scalp and lower the chance of passing infection to others, but it is usually an addition rather than the only treatment. Improvement in scaling may come before hair looks normal again.

Take the oral antifungal exactly as prescribed and attend any follow-up requested. Use the medicated shampoo according to its instructions, avoiding contact with the eyes. Do not share combs, brushes, hats, pillows or towels. Wash or clean items that touch the scalp as advised, and ask close household contacts to seek assessment if they have scalp changes.

Avoid scratching or pulling affected hairs. A clinician may examine the scalp or arrange testing when the appearance does not clearly identify the fungus.

Seek prompt care for a tender swollen area, pus, extensive crusting, fever or rapidly worsening inflammation. These features can signal a stronger inflammatory reaction and need timely treatment. Review is also needed for persistent scaling, broken hairs or patchy hair loss despite treatment.

Ask which oral antifungal is being considered, how it should be taken, whether liver or medicine checks are needed, how often to use shampoo, and whether family members or pets need assessment.
