What are the treatment options for cervical myelopathy?

Cervical myelopathy means the spinal cord is being compressed in the neck. Disc changes, arthritis, or narrowing around the spinal canal can reduce space for the cord. Assessment usually combines symptoms, strength, reflexes, walking pattern, hand coordination, and imaging. Increasing clumsiness or weakness makes timely spinal review especially important.

A laminectomy removes part of the back of the vertebra to make more room for the cord. Cervical fusion may be added when movement between vertebrae, alignment, or front-sided compression needs stabilising. The operation selected depends on the location and shape of compression and the condition of the neck.

Arrange a clinical assessment if you notice hand clumsiness, dropping objects, balance trouble, or a change in walking. A specialist may recommend observation with planned review when findings are mild and unchanged. Physiotherapy can focus on safe movement, strength, balance, and function, but it should be tailored when the spinal cord is involved.

Follow the activity and medication advice given by your clinician. Avoid delaying review because symptoms fluctuate; a careful comparison of strength, reflexes, and walking over time helps reveal progression.

Seek urgent medical care for rapidly worsening weakness, new difficulty walking, repeated falls, loss of hand function, or new bladder or bowel control changes. After a neck injury, new limb weakness or altered sensation also needs prompt assessment.
