What the combination may mean

Fatigue means reduced energy or stamina, not simply feeling sleepy. Weight can change when appetite, movement, sleep, stress, or fluid balance changes. Anaemia may cause tiredness and breathlessness; coeliac disease can affect digestion and nutrient absorption; chronic kidney disease may cause fatigue and swelling. Alcohol use disorder, burnout, chronic fatigue syndrome, COVID-19, and dehydration can also produce fatigue, although each has a different pattern.

The timing helps. Tell your clinician whether the change followed an illness, medicine, mood change, altered sleep, menstrual or hormonal change, or a shift in appetite. A review may include an examination, medication check, and selected blood or urine tests.

Seek urgent medical help for chest pain, severe breathlessness, fainting, new confusion, one-sided weakness, black or bloody stools, inability to keep fluids down, or rapidly increasing swelling. Prompt review is also sensible when fatigue is accompanied by persistent fever, significant bleeding, or a sudden major change in weight.
