What vestibular headache can feel like

The word vestibular refers to the balance system and the way the brain processes movement. Symptoms may include spinning or swaying, unsteadiness, sensitivity to busy patterns or screens, and discomfort in a moving car. Head pain may be throbbing, one-sided or felt as pressure, but some episodes are mainly dizziness. Light or sound sensitivity and nausea can occur alongside an episode.

Episodes can be brought on by poor sleep, skipped meals, stress, dehydration, hormonal changes, strong smells or visually demanding environments. Other illnesses can also cause headache or dizziness, so a clinician may consider the timing, pattern, associated symptoms and examination rather than relying on one symptom alone.

What you can do

During symptoms, sit or lie somewhere quiet and dim, move your head slowly, and avoid driving or climbing until your balance has returned. Sip fluids if you can, eat regularly, and record possible triggers, duration, activities and accompanying symptoms. A regular sleep routine and gradual activity may help reduce attacks.

Arrange a medical assessment if episodes recur, interfere with work or walking, or are changing. Treatment may involve a clinician-selected medicine for attacks, preventive treatment, or balance-focused rehabilitation. Do not repeatedly increase over-the-counter pain medicine without advice, because frequent use can itself worsen headache patterns.

When to seek urgent care

Seek emergency help for a sudden, severe headache unlike your usual episodes, new weakness or numbness, trouble speaking, fainting, confusion, a new loss of vision, inability to walk, repeated vomiting, fever with a stiff neck, or symptoms after a significant head injury. Get prompt medical advice for a first or rapidly worsening episode, especially if you are pregnant or have a condition that affects immunity or blood clotting.
