Very painful ovulation pain: what to know

Typical ovulation pain is often one-sided and occurs around the middle of a cycle, but severe pain can have other explanations. Endometriosis may cause recurring pelvic pain, painful periods, pain during sex, or bowel and bladder symptoms. An ovarian cyst can cause pressure or sudden pain, particularly if it ruptures or twists the ovary. Pregnancy-related causes, infection, appendicitis, or urinary problems can also mimic ovulation pain.

The timing alone cannot confirm ovulation as the cause. Note the side, onset, duration, cycle day, bleeding, discharge, fever, vomiting, urinary symptoms, and possibility of pregnancy. A clinician may use an examination, pregnancy testing, or ultrasound to investigate.

Pause strenuous activity if movement worsens the pain and use gentle heat if it feels soothing. Keep a symptom and cycle diary to show whether the pain repeats at a similar time. Ask a clinician which pain relief is suitable for you, especially if pregnancy is possible. Recurrent pain deserves review even when it settles between episodes.

Seek urgent care for sudden severe pain, fainting, heavy bleeding, fever, repeated vomiting, a swollen abdomen, or pain with a positive pregnancy test. These features can signal a problem needing rapid treatment, including an ovarian emergency or ectopic pregnancy.
