Short answer

A very heavy period after childbirth needs attention because bleeding may reflect hormonal recovery, anaemia, or a condition requiring treatment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

After birth
The return of periods can be irregular, especially during breastfeeding.
Watch for
Flow, clots, pain, fever, dizziness, and breathlessness.
Reason to check
Heavy bleeding can contribute to iron deficiency anaemia.

What it may mean

Periods can take time to return after pregnancy, particularly when breastfeeding. Early cycles may be irregular or heavier as ovulation resumes, but postpartum bleeding and a returned period are different. Bleeding soon after birth, bleeding that becomes heavier after it had eased, or bleeding with fever and pelvic pain may have a different cause.

A heavy period is a common presentation of several womb conditions, including fibroids, adenomyosis, polyps, and an overgrown womb lining. Polycystic ovary syndrome and perimenopause can also disturb cycle timing. Blood loss may lead to iron deficiency anaemia, causing tiredness, headaches, breathlessness, or paleness.

What to do next

Contact a clinician or maternity service to describe the timing, flow, clots, pain, fever, and whether you are breastfeeding. Mention medicines, contraception, and any pregnancy possibility. Assessment may include checking blood pressure, examining the abdomen or pelvis, and blood tests for anaemia; an ultrasound may be used to examine the womb.

Use the recommended menstrual protection and record how often it needs changing. Avoid self-prescribing hormonal medicines or iron until a clinician has considered breastfeeding, other medicines, and the cause of bleeding.

When to seek care urgently

Get emergency help for rapidly increasing bleeding, faintness, confusion, severe weakness, shortness of breath, chest discomfort, severe abdominal or pelvic pain, fever, or feeling very unwell. These symptoms can accompany major blood loss or infection and should not wait for a routine appointment.

Questions for your doctor

Is this a returned period or postpartum bleeding? Could anaemia be contributing to my symptoms? Do I need an examination, blood tests, or an ultrasound? Which treatments are compatible with breastfeeding and my contraception? What flow or symptoms should trigger urgent help?

Questions people ask

How can I tell postpartum bleeding from a period?

Postpartum bleeding begins after birth and usually changes as the body heals; a period returns later, although timing varies. New heavy bleeding should be discussed with a maternity or medical team.

Can I take iron for heavy bleeding after birth?

Ask a clinician first. They may recommend a blood test and select an iron plan based on your result, symptoms, medicines, and breastfeeding.
