Very heavy period after birth: what should you know?

Vaginal bleeding after delivery is called lochia and gradually changes as the uterus heals; it is not the same as a menstrual period. The first periods after cycles return can be irregular or heavier, and breastfeeding may delay or alter ovulation. Heavy bleeding can also relate to retained pregnancy tissue, infection, changes in the uterine lining or conditions such as Uterine Fibroids, Adenomyosis or Endometrial Polyps. A new pregnancy is possible before a first period, so pregnancy should be considered when symptoms fit.

Note when delivery occurred, whether bleeding had previously eased, how quickly products are becoming saturated, and whether you have clots, pain, fever, dizziness or foul-smelling discharge. Contact your maternity team or clinician for advice about new or worsening heavy bleeding. They may examine you and arrange blood tests or an ultrasound. Mention breastfeeding, contraception, medicines and any pregnancy possibility because these details affect assessment and treatment.

Seek emergency help for rapidly increasing bleeding, faintness, collapse, racing heartbeat, severe abdominal pain, fever with feeling very unwell, or pale, clammy skin. Do not travel alone if dizzy. Heavy bleeding shortly after delivery is not judged by whether it resembles a period; the speed of blood loss and your symptoms matter. If heavy cycles continue later, arrange review for possible Iron Deficiency Anaemia and other causes.

Ask whether the bleeding is lochia, a returning period or another problem, whether you need a blood count or iron test, and which examination or imaging is appropriate. Discuss contraception, breastfeeding-compatible treatments and the warning signs that require immediate help.
