Why bleeding and pain can occur together

Strong cramps happen when the uterus contracts, but severe or changing pain deserves attention. Heavy painful periods may occur with Adenomyosis, Uterine Fibroids or Endometrial Polyps. Endometrial Hyperplasia and hormone-related ovulation changes, including Polycystic Ovary Syndrome, can affect bleeding without explaining every pain pattern. Perimenopause can also bring cycle changes. Blood loss may contribute to Iron Deficiency Anaemia, while pain severity may be influenced by inflammation, uterine changes or another pelvic condition.

Steps that help a clinician understand the pattern

Record when pain begins, where it is felt, whether it radiates to the back or legs, and how bleeding changes from cycle to cycle. Note clots, bleeding between periods, pain during sex, bowel or bladder symptoms, fever and medicines you have tried. Heat and rest may help some people, but use pain medicines only as directed and check suitability if you have stomach, kidney, bleeding or pregnancy concerns. A clinician may arrange blood tests and pelvic imaging.

When to seek urgent care

Get urgent medical help for sudden severe pelvic pain, fainting, heavy bleeding with weakness, fever with pelvic pain, repeated vomiting, or possible pregnancy with pain or bleeding. These features can signal a problem needing rapid assessment. Seek review when pain regularly prevents work, sleep, movement or usual activities, even if the bleeding is familiar.
