What should you know about very early miscarriage symptoms?

A very early miscarriage may involve spotting or bleeding that becomes heavier, lower abdominal cramps, pelvic pressure, or passing clots or tissue. Some people notice a reduction in pregnancy symptoms, while others have no clear symptoms at first. Bleeding and cramps can also occur in an ongoing pregnancy, so symptoms alone cannot confirm a loss.

Assessment may include questions about the bleeding and pain, pregnancy testing over time, and a Transvaginal Ultrasound when the timing makes it useful. The clinician may also consider an ectopic pregnancy, which can be dangerous and may present with one-sided pain, shoulder-tip pain, dizziness, or faintness.

Contact your maternity or gynaecology service if you have bleeding or pain after a positive pregnancy test. Note the amount and colour of bleeding, clots or tissue, pain location, and any dizziness or fever. Use pads rather than tampons while bleeding so you can monitor it, and follow the service's advice about pain relief. Do not blame yourself: everyday activity, work, or ordinary emotional stress does not explain most early losses.

Go to urgent care for severe or one-sided abdominal pain, shoulder-tip pain, faintness, heavy bleeding, weakness, fever, or feeling seriously unwell. These symptoms need prompt assessment because ectopic pregnancy, significant bleeding, or infection may require immediate treatment. If symptoms are mild but continue, contact your pregnancy care team promptly rather than waiting for the next routine visit.

Ask whether you need repeat pregnancy tests or an ultrasound, what bleeding pattern should prompt urgent help, when you can resume usual activities, and which treatment options would be appropriate if a miscarriage is confirmed.
