Vancomycin rash: what to know

A reaction during or soon after an infusion may cause flushing, itching, redness, or raised areas, especially on the upper body. A delayed drug eruption can appear later and may spread beyond the original area. The appearance alone cannot establish the cause: infection, another medicine, or a separate condition such as [Drug Rash](/conditions/drug-eruption/) may look similar.

The medication team needs the dose timing, infusion details, other medicines, and symptoms such as fever, wheezing, dizziness, mouth sores, or skin pain. Do not assume a new rash is [Atopic Dermatitis](/conditions/atopic-dermatitis/), [Contact Dermatitis](/conditions/contact-dermatitis/), [Heat Rash](/conditions/heat-rash/), [Intertrigo](/conditions/intertrigo/), [Psoriasis](/conditions/psoriasis/), [Ringworm](/conditions/ringworm/), or [Scabies](/conditions/scabies/) without an examination.

Tell the nurse, prescriber, or pharmacist as soon as you notice a rash. Record when it began, where it started, whether it followed an infusion, and whether it is spreading. Photographing the pattern can help if it changes before review. The treating team may decide whether to pause, adjust, or replace treatment; do not make that decision alone when vancomycin is treating a serious infection.

Use only medicines recommended for the reaction, and avoid applying new creams that could obscure the rash. Keep the skin cool and avoid scratching. Give clinicians a list of previous antibiotic reactions, including what happened and how quickly it developed.

Seek emergency help for trouble breathing, throat or tongue swelling, faintness, wheezing, or rapidly spreading hives. Urgent assessment is also needed for blistering, peeling skin, severe skin pain, purple spots, eye involvement, mouth or genital sores, or a rash with fever and marked illness. These features can signal a serious reaction and should not wait for a routine appointment.

Ask whether the pattern fits an infusion reaction or delayed drug eruption, which medicine is most suspected, whether vancomycin should be recorded as an allergy, and what alternatives are available. Confirm what symptoms require emergency help and whether future doses need a different monitoring plan.
