Can a UTI Cause Flank Pain?

Flank pain is felt between the ribs and the hip, often on one side of the back. A bladder UTI more often causes burning while passing urine, frequent urges, or lower abdominal discomfort. Pain higher in the side or back, together with fever or feeling unwell, can occur when infection involves the kidney. A kidney infection needs medical assessment because it can worsen without treatment.

Flank pain has other possible causes, including a kidney stone, muscle strain, or problems affecting nearby organs. Severe pain that comes in waves, blood in the urine, or difficulty passing urine can fit with a stone, but symptoms overlap. A clinician may use your symptoms, an examination, and a urine test to help identify the cause.

Arrange prompt medical advice if you have flank pain with urinary symptoms, fever, chills, vomiting, or marked weakness. Drink fluids normally unless a clinician has told you to restrict them, and do not use leftover antibiotics. A clinician may recommend an antibiotic after assessing whether the infection is likely and whether testing is needed. Take any prescribed medicine exactly as directed and seek further advice if symptoms do not start improving.

Seek urgent medical care for severe or rapidly worsening flank pain, repeated vomiting, confusion, fainting, trouble breathing, or inability to keep fluids down. Pregnancy, a weakened immune system, known kidney disease, or difficulty passing urine lowers the threshold for urgent assessment. Do not wait for urinary symptoms to become obvious if you feel seriously unwell.
