Uterine Bleeding After Sex: What You Should Know

Although people may call this uterine bleeding, blood noticed after intercourse may come from the vaginal walls or cervix. A small amount can follow friction or a minor surface injury, but repeated bleeding deserves a clinical assessment even when there is no pain.

Dryness, vigorous friction, or a tear can make delicate vaginal tissue bleed. Hormonal changes around menopause can thin and irritate the vaginal and urinary tissues; this pattern is associated with Genitourinary Syndrome of Menopause.

Inflammation or infection of the cervix may also cause contact bleeding. Chlamydia is one infection that can present with bleeding after sex, sometimes alongside unusual discharge, pelvic discomfort or pain when urinating. Cervical dysplasia and cervical cancer can present with bleeding after sex, so screening history and examination matter. Less commonly, a vaginal growth or vaginal cancer can be involved. These possibilities do not mean that bleeding itself identifies a particular condition.

Note when the bleeding happened, how much there was, whether it followed penetration, and whether you had pain, discharge, fever or bleeding between periods. Avoid further penetration until soreness or bleeding has settled, and use a water-based lubricant when you resume if dryness may be contributing.

Arrange an appointment with a gynaecologist or primary-care clinician if the bleeding happens again, continues, or has no clear explanation. They may review your menstrual and pregnancy history, medicines and screening, then examine the vagina and cervix. Tests can include a pregnancy test, infection testing or cervical evaluation. Do not douche or place products in the vagina before assessment.

Seek urgent medical help for heavy or rapidly increasing bleeding, faintness, severe pelvic or abdominal pain, shoulder-tip pain, fever, or feeling very unwell. If pregnancy is possible, bleeding with pain, dizziness or weakness needs prompt assessment because pregnancy-related causes require timely care.

A clinician should also assess bleeding after sex that occurs after menopause, follows a recent procedure, or comes with persistent pelvic pain or an abnormal-smelling discharge.

Ask: Could the blood be coming from the vagina or cervix? Do I need an examination, infection test or cervical screening? Could dryness, a medicine or contraception be contributing? Which symptoms should prompt urgent help, and when should I follow up if the bleeding returns?
