Ustekinumab and ankylosing spondylitis: what to know

Ankylosing spondylitis is an inflammatory condition that can affect the spine and the joints where the pelvis and spine meet. Joint stiffness, morning joint stiffness, and reduced range of motion may occur, although symptoms vary. Ustekinumab changes selected immune signals and is used for some inflammatory diseases, but a medicine that helps one condition may not control spinal inflammation in another.

A rheumatologist weighs the diagnosis, imaging, symptoms, other inflammatory features, previous medicines, infections, and health history. The name of a medicine alone cannot show whether it is suitable. Treatment decisions should distinguish active axial disease from mechanical back pain or another cause of stiffness.

Ask a rheumatologist to confirm whether your symptoms and imaging fit ankylosing spondylitis and whether ustekinumab has a specific reason in your case. Keep a record of morning stiffness, night pain, activity limits, eye symptoms, bowel symptoms, skin changes, and medicine effects. Do not start, stop, or space biologic treatment without the prescribing team.

Movement usually forms part of long-term care. A tailored physiotherapy plan may include posture, spinal mobility, breathing, strength, and pacing. Exercise therapy or hydrotherapy can be considered according to pain, mobility, and fitness. Anti-inflammatory medication may be discussed when appropriate, with attention to stomach, kidney, heart, and other risks.

Contact the prescribing team promptly for fever, a persistent cough, painful urination, a new skin infection, or other signs of infection while using an immune-modifying medicine. Seek urgent care for severe eye pain or light sensitivity, sudden leg weakness, loss of bladder or bowel control, or rapidly worsening back pain after injury. A flare with substantial loss of movement also needs timely review.
