Unusual Bleeding After Sex: What to Know

Unusual bleeding after sex should be assessed if it is new, repeated, heavy, or accompanied by pain, discharge, or pregnancy concerns. A small amount of spotting can follow friction, a minor vaginal tear, or dryness, but bleeding that keeps happening deserves a clinical explanation. The timing, amount, colour, relation to your menstrual cycle, and other symptoms can help a clinician identify the source.

Bleeding may come from irritated vaginal tissue, reduced lubrication, a vaginal infection, or inflammation around the cervix. Chlamydia can cause bleeding after sex, pelvic discomfort, or unusual discharge, and some people have few symptoms; testing is needed to identify it. Around and after menopause, genitourinary syndrome of menopause can make vaginal and urinary tissues thinner and drier, which may contribute to soreness and spotting.

Cervical dysplasia and cervical cancer can present with bleeding after sex, so recurring or unexplained episodes should not be dismissed. Vaginal cancer is another uncommon cause. These possibilities do not mean that bleeding itself identifies a cancer diagnosis; examination and appropriate tests are what clarify the cause.

Arrange an appointment with a gynaecologist or primary-care clinician when the bleeding is unexplained, happens again, or occurs after menopause. Note when it started, whether it followed penetration, how much blood there was, and whether you have pain, itching, fever, discharge, urinary symptoms, or a missed period. Avoid sex until heavy bleeding or significant pain has settled, and use a pad if you need to monitor the amount. A clinician may discuss a pregnancy test, infection testing, a pelvic examination, cervical screening history, or imaging depending on your symptoms.

Do not self-treat with leftover antibiotics or vaginal products before assessment, because they can irritate tissue or make infection testing harder. If a cervical abnormality needs treatment, a cone biopsy is one procedure a specialist may discuss; it is not appropriate for every cause of bleeding.

Seek urgent medical help for heavy or rapidly increasing bleeding, severe or one-sided pelvic pain, faintness, marked weakness, fever, or shoulder-tip pain. If you could be pregnant, bleeding with pelvic pain, dizziness, or fainting needs prompt assessment because pregnancy-related problems can require urgent treatment. Contact a clinician promptly for bleeding after menopause, bleeding after sex that keeps recurring, or bleeding with foul-smelling discharge or pelvic pain.

Ask what may be causing the bleeding and whether you need a pelvic examination, pregnancy test, infection testing, cervical screening, or imaging. You can also ask whether dryness, medicines, contraception, or a recent procedure could be contributing, what warning signs should bring you back sooner, and when to expect follow-up if testing finds a cervical change.
