Unilateral hydrosalpinx: what to know

A unilateral hydrosalpinx is one fallopian tube blocked and expanded by fluid, and it needs individual fertility and treatment planning. The other tube may remain open, but its condition and the cause of the blockage matter.

Inflammation or scarring can seal a fallopian tube, allowing fluid to collect inside it. Pelvic inflammatory disease is one possible cause; previous pelvic infection, surgery or endometriosis may also contribute. Some people have pelvic pain, pressure or unusual discharge, while others discover the condition during fertility tests. Imaging may suggest a hydrosalpinx, but a specialist interprets the result alongside your history and other fertility findings.

The fluid-filled tube can interfere with the movement of egg and sperm and may affect the womb environment if fluid drains back toward it. This does not describe the function of the unaffected tube, so decisions are not based on the scan finding alone.

Ask for a gynaecology or fertility assessment to review the tube, ovarian reserve, sperm factors, previous infections and pregnancy goals. Treatment may involve monitoring, surgery to remove the affected tube, or assisted reproduction such as IVF. A salpingectomy removes a fallopian tube; whether it is suitable depends on your anatomy, symptoms, future plans and the health of the other tube. Treating active infection, when present, is a separate part of care.

Seek urgent help for sudden or severe one-sided pelvic pain, faintness, fever, vomiting or heavy bleeding. These symptoms can accompany infection, twisting of an adnexal structure or another urgent pelvic problem. Contact a clinician promptly if pain, fever or foul-smelling discharge develops after a procedure.

Ask how certain the diagnosis is, whether the other tube is open, and whether infection or endometriosis is suspected. Ask how each option affects natural conception, IVF planning, ovarian function and recovery, and what symptoms should bring you back sooner.
