Undulant refers to a rise and fall, like a wave. It describes what the fever does rather than naming its cause. Chills, sweating, tiredness, headache, appetite loss, and joint or muscle aches can accompany the pattern. Brucellosis is one possibility, particularly with animal or unpasteurised dairy exposure, but a clinician must consider the wider picture.

A cough, sore throat, rash, or breathing change may point toward another infection, including a common cold, COVID-19, chickenpox, or acute bronchitis. The timing of symptoms and any exposure often matter as much as the temperature itself.
