What should you know about ulcerative colitis left side pain?

The left side of the abdomen overlies parts of the large bowel. In a person with ulcerative colitis, inflammation or bowel spasm can present with cramping, urgency, diarrhoea, mucus or blood. Some people notice pain before passing stool, while others have a more constant ache. The pain pattern does not confirm that ulcerative colitis is active.

Constipation, trapped gas, infection, urinary problems and conditions outside the bowel can also cause left-sided pain. The clinician will consider the pain’s onset, intensity, movement, relation to meals or bowel movements, and any recent change in usual symptoms.

Contact your GP or ulcerative-colitis team if the pain is new, lasts longer than expected, keeps returning or occurs with a change in stool frequency or bleeding. Write down the timing, location, severity, bowel movements, temperature and medicines taken. Continue prescribed treatment unless your clinician gives different instructions.

A review may involve an abdominal examination and tests chosen for your symptoms. Do not rely on pain relief to conceal worsening symptoms, and avoid using anti-inflammatory pain medicines or laxatives unless a clinician says they are suitable for you.

Seek urgent help if left-sided pain is severe, rapidly worsening or accompanied by a rigid or swollen abdomen, repeated vomiting, fainting, high fever, inability to pass stool or gas, or significant rectal bleeding. Urgent assessment is also appropriate when pain feels very different from your usual flare symptoms.

Ask whether the pain pattern suggests active bowel inflammation, what other causes should be checked, which pain medicines are appropriate, and how to recognise a flare. Clarify who to contact outside appointment hours and when testing or specialist review is needed.
