Ulcerative colitis and bowel movements: what to know

Inflammation in the bowel lining can make the rectum more sensitive, creating urgency even when little stool passes. Movements may be loose, frequent, or mixed with mucus and blood. Some people have cramping before using the toilet, night-time symptoms, or tenesmus, which is the sensation that more stool remains. A sudden change can also come from infection, food intolerance, medicines, or constipation with overflow, so symptoms need to be considered as a pattern rather than judged by one stool.

Record frequency, urgency, stool texture, visible blood or mucus, pain, fever, night-time waking, and any recent medicine or diet changes. Drink regularly, protect irritated skin with gentle washing, and keep taking prescribed colitis treatment unless your clinician advises otherwise. Contact your GP or IBD team when your usual pattern changes, symptoms persist, or you are struggling to work, sleep, eat, or drink. They may use blood or stool tests to assess inflammation and infection.

Seek urgent help for severe or escalating abdominal pain, a swollen abdomen, repeated vomiting, faintness, confusion, very little urine, or inability to keep fluids down. Heavy rectal bleeding, high fever, or rapidly worsening diarrhoea also needs prompt assessment. These warning signs matter regardless of how common a symptom is in your usual colitis pattern.
