Types of liquid biopsy

The most familiar form analyses fragments of genetic material released by tumour cells into the bloodstream. This is often called circulating tumour DNA testing. Another approach looks for intact circulating tumour cells, which have separated from a tumour and entered the blood. Some tests examine other tumour signals, such as RNA or proteins, although the available test and its purpose depend on the clinical question.

A liquid biopsy is different from a tissue biopsy. It does not remove a piece of the tumour, so it may not show the tumour’s full structure or every change within it. A blood result can help identify a treatment-related alteration, monitor changes over time, or provide information when tissue is difficult to obtain. A negative result may not exclude cancer or a molecular change because the sample may contain too little tumour material.

Ask what the test is designed to answer before the sample is taken. Your clinician may combine the result with scans, symptoms, examination findings, and tissue pathology. If a treatment decision depends on a genetic finding, the team may recommend confirming the finding with tumour tissue when possible. Follow the instructions for the blood draw and share medicines, recent procedures, and any previous pathology reports.

Contact your oncology team promptly if new breathlessness, chest pain, coughing blood, faintness, or rapidly worsening weakness develops. These symptoms need clinical assessment whether or not a liquid biopsy has been planned. Seek urgent help for severe breathing difficulty, heavy bleeding, confusion, or collapse.
