Twin pregnancy with one molar pregnancy

A molar pregnancy is abnormal placental tissue. In a twin pregnancy, one fetus may be developing alongside that tissue. The molar component can release high levels of pregnancy hormone and may be linked with vaginal bleeding, strong nausea, pelvic pressure or signs of thyroid overactivity. A symptom alone cannot establish the diagnosis.

Ultrasound, examination and serial hormone testing help the team understand the pregnancy. Care may involve maternal-fetal medicine, obstetrics and specialists in gestational trophoblastic disease. The plan depends on the scan findings, your symptoms, your health and fetal development.

Ask for a clear specialist follow-up plan and attend each scan and blood-test appointment. Record bleeding, pain, vomiting, dizziness, palpitations and changes in fetal movement, then report them to the maternity team. Avoid self-treating with medicines, including herbal products, until the team has checked them.

Your clinicians may explain options for pregnancy monitoring, timing of delivery and follow-up after birth or pregnancy loss. If active molar tissue persists, chemotherapy may be considered after specialist assessment.

Go to emergency care for heavy bleeding, collapse, severe or one-sided abdominal pain, chest pain, breathing difficulty, a seizure or severe headache with visual symptoms. Call your maternity service promptly for persistent vomiting, dehydration, fever or any concern about fetal movement.

Ask how the molar pregnancy was identified, what the ultrasound can show, which hormone tests are planned, how the fetus will be monitored, what delivery choices may arise, and how long follow-up will continue.
