Troponin blood test: what the result means

Blood is usually taken from a vein in your arm. The clinician may repeat the test because troponin can change over time after an injury. The test is one part of an urgent assessment, not a stand-alone diagnosis.

A raised troponin level can indicate heart muscle injury. A heart attack is one possible cause, but inflammation of the heart, an abnormal heart rhythm, heart failure, a blood clot in the lungs, severe infection or reduced kidney function can also affect the result.

A result within the expected range does not by itself explain every type of chest discomfort, and timing matters. Clinicians compare the level with the laboratory range, symptoms, an electrocardiogram and sometimes scans or further blood tests.

Chronic Kidney Disease can affect how troponin is interpreted, so tell the team about known kidney problems and recent illness. Anaemia and other conditions may also be considered when assessing breathlessness, weakness or chest symptoms.

Follow the instructions from the service arranging the test. Tell the clinician about chest pain, breathlessness, fainting, sweating, nausea, recent strenuous activity, medicines and any heart or kidney history. Most people can use the arm normally after the sample, although mild bruising can occur.

Do not use a troponin result to decide on treatment yourself. Ask when the result will be reviewed and what other findings will be used alongside it. If the test was arranged during an emergency visit, follow the discharge advice even if you feel better.

Seek emergency help immediately for pressure, tightness or pain in the chest that does not settle, or for severe breathlessness, fainting, sudden sweating, marked weakness, or pain spreading to the arm, back, neck or jaw. Do not drive yourself if you feel seriously unwell.

If you have already had a troponin test and symptoms return or worsen, contact emergency services rather than waiting for a routine appointment. A test result needs prompt clinical context when heart symptoms are possible.
