Trimodal therapy for bladder cancer

The first part is a thorough transurethral procedure, in which visible tumour is removed through a cystoscope and tissue is sent for pathology. Chemotherapy is then used to make cancer cells more sensitive to radiation and to treat cells beyond the treated area. Radiotherapy targets the bladder and planned nearby regions over a scheduled course.

This approach is considered only after careful staging and bladder assessment. Tumour location, depth, bladder function, urine symptoms, ability to attend treatment, and general health all matter. Some people may instead be advised to have bladder removal surgery, while others need a different treatment pathway.

Ask the multidisciplinary team how the pathology and scans support the recommendation, what bladder function is expected, and how treatment could affect urination, bowel movements, fatigue, or sexual health. Tell the team about difficulty passing urine, infections, kidney problems, medicines, and previous pelvic treatment.

Bladder-preserving treatment requires close follow-up, often including cystoscopy, urine checks, and imaging when indicated. A new bladder finding may need another biopsy or treatment discussion. Intravesical BCG therapy and immunotherapy are different approaches and may be relevant in other bladder-cancer settings, not automatically part of trimodal therapy.

Seek urgent help for inability to pass urine, heavy bleeding or clots, fever with chills, severe pelvic or abdominal pain, persistent vomiting, or rapidly worsening breathlessness. During chemotherapy or radiotherapy, report fever, severe diarrhoea, dehydration, or new confusion promptly because treatment may need adjustment.
