Treatment for shortness of breath due to heart failure

When the heart pumps less effectively, pressure can build behind it and fluid may collect in or around the lungs. Breathlessness may be worse when lying flat, wake you at night, or come with ankle swelling, fatigue, or rapid weight change. Asthma, COPD, anaemia, angina, atrial fibrillation, and lung infections can cause similar symptoms, so a clinician may examine you and arrange heart and lung tests rather than assuming every episode is heart failure.

A clinician may adjust medicines that remove excess fluid or reduce strain on the heart, while treating triggers such as an abnormal rhythm or infection. Take medicines on schedule and never change a diuretic dose without advice. Record breathlessness, swelling, sleep position, and any sudden change in your usual symptoms. Follow the agreed plan for fluids, salt, activity, and monitoring; the right limits depend on your heart function and other health conditions.

Call emergency services for severe or sudden breathlessness, blue or grey lips, chest pressure, fainting, new confusion, coughing pink froth, or inability to speak comfortably. Seek urgent review when breathlessness is worsening, appears at rest, follows missed medicine, or comes with new swelling or a rapid change in your usual weight. Do not drive yourself if you feel faint or severely short of breath.

Ask which symptoms suggest fluid overload, how to take or monitor your medicines, what daily changes should prompt a call, and whether another condition could be contributing. Confirm your follow-up plan and the tests needed to assess heart and lung function.
