Treatment for gout with kidney disease

Gout involves urate crystals causing inflammation in a joint, while the kidneys help remove urate and many medicines from the body. Reduced kidney function can alter medicine handling and may affect which anti-inflammatory option is suitable. A flare may present with joint pain, warmth, redness, and swelling, but infection can look similar and must not be assumed away.

Tell the clinician about your kidney diagnosis, recent kidney results, fluid limits, allergies, and every medicine or supplement you use. They may consider a corticosteroid or carefully selected colchicine plan; non-steroidal anti-inflammatory medicines may be unsuitable for some people with kidney disease. Do not take over-the-counter anti-inflammatory tablets, double a dose, or stop prescribed kidney or gout medicine without advice. Maintain the fluid plan given by your kidney team rather than forcing extra fluids. Once the flare settles, ask about urate-lowering treatment and monitoring to reduce future attacks.

Seek urgent care for fever, chills, rapidly worsening joint redness, pus, confusion, severe weakness, very little urine, or breathing difficulty. Contact your care team promptly if you cannot keep medicines down, develop new swelling, or the flare is not responding. A hot swollen joint may be infected even in someone who has had gout before.
