Treatment for bladder cancer in older adults

Age alone does not determine treatment. The team may assess walking ability, memory, nutrition, kidney function, heart and lung health, independence and support at home. Depending on the findings, care may involve removal through the bladder, [Intravesical BCG Therapy](/treatments/systemic-therapy/intravesical-bcg-therapy/), [Chemotherapy](/treatments/systemic-therapy/chemotherapy/), [Immunotherapy](/treatments/systemic-therapy/immunotherapy/) or [Bladder Removal Surgery](/treatments/surgical-oncology/cystectomy/). A geriatric or supportive-care review can help balance cancer control with independence and comfort.

Bring a complete medicine list, including non-prescription products, and mention falls, hearing or vision problems, memory changes and help available at home. Ask about transport, hydration, nutrition, catheter care and who to call outside clinic hours. A family member or trusted person can help record instructions if you wish.

Treatment plans can be adjusted when kidney function, frailty or another illness changes the balance of benefit and burden. Discuss the goals openly: controlling visible cancer, reducing future bladder problems, preserving function or focusing on comfort may lead to different choices.

Contact the care team for fever, chills, worsening pain, new confusion, repeated vomiting, heavy blood in the urine or inability to pass urine. Seek emergency care for severe weakness, fainting, difficulty breathing, uncontrolled bleeding or sudden confusion. Older adults can become unwell quickly, so a new change from usual function deserves attention.

Ask what stage the bladder cancer has reached, what each option is intended to achieve, how it may affect independence, and how kidney function or other medicines change the plan. Ask whether a treatment pause, shorter course, bladder-preserving approach or supportive-care referral is suitable.
