Why treatment needs individual adjustment

Sepsis can make blood pressure fall and place extra demand on the heart. Heart failure can make it harder for the body to tolerate large fluid volumes, so clinicians assess breathing, blood pressure, urine output, oxygen levels and signs of congestion repeatedly. Treatment may include intravenous antibiotics, oxygen when needed, fluids in measured amounts, and medicines that support blood pressure if circulation remains unstable. Regular heart-failure medicines may need temporary review, especially if blood pressure or kidney function changes. A chest infection, urinary infection or another source may need testing and treatment.

What to do while help is arranged

Sepsis is an emergency: call emergency services if a person with heart failure is suddenly confused, very breathless, faint, unusually cold or clammy, or difficult to wake. Do not delay for a routine appointment. Keep an up-to-date medicine list available, including diuretics and heart medicines, and tell clinicians about fluid restrictions, kidney problems, allergies and recent infections. Do not give extra fluids or change prescribed doses unless a clinician instructs you. If safe, note when symptoms began and whether urine output, breathing or swelling has changed.

When urgent care is needed

Seek emergency help immediately for rapid breathing, blue or grey lips, new confusion, collapse, severe weakness, chest pressure, or a marked drop in responsiveness. Fever is not required for sepsis, and older adults may show confusion or a sudden functional decline instead. Worsening breathlessness or rapidly increasing swelling can also signal fluid overload during an infection. After discharge, contact the treating team promptly for returning fever, reduced urine, vomiting that prevents medicines, or symptoms that are getting worse rather than settling.

Questions to ask the treating team

Ask what infection is suspected, which medicines should continue, how fluid intake is being managed, and which warning signs mean returning to hospital. Clarify when kidney function, weight and breathing should be checked after discharge, and request a written plan for restarting any temporarily held medicine.
