Short answer

Tranexamic acid can generally be taken with or without food, but follow the prescribed directions and use food if it reduces stomach upset.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Food
Often taken with or without food.
Water
Swallow doses with water unless instructed otherwise.
Key caution
Report clot symptoms or new vision changes urgently.

What taking it on an empty stomach means

An empty stomach is not automatically required for tranexamic acid. The medicine helps slow the breakdown of clots, so the reason for taking it matters: a clinician may prescribe it for heavy menstrual bleeding, another bleeding problem, or a skin condition. The tablet or liquid instructions can differ from one product to another. Food may make the dose easier to tolerate if you notice nausea, abdominal discomfort, or heartburn. Do not crush or alter a formulation unless the label or pharmacist says that is suitable.

How to take your dose

Take each dose at the prescribed time with water. If your stomach feels unsettled, taking it during or after a meal may help, unless your prescriber gave a specific fasting instruction. Do not take extra doses after a missed dose without checking the instructions. Tell your clinician about kidney problems, previous blood clots, vision changes, or medicines containing hormones, because these details can affect whether tranexamic acid is appropriate. Keep the medicine away from someone else’s treatment plan.

When to seek care

Seek urgent medical help for sudden chest pain, difficulty breathing, one-sided leg swelling or pain, coughing blood, or a new change in vision. Contact a clinician promptly if bleeding is heavy, continues despite treatment, returns unexpectedly, or causes faintness and marked weakness. Vomiting that prevents you from keeping doses down also deserves advice, because missed treatment may matter for the condition being treated.

Questions to ask

Ask whether your exact product should be taken with meals, what to do after a missed dose, and how long the course is intended to last. You can also ask which symptoms mean you should stop the medicine and seek urgent care.
