Topiramate Skin Rash: What to Know

A rash can be flat, raised, itchy, scaly, blistered, or painful. It may result from irritation, heat, infection, eczema, psoriasis, or contact with a new product. A medication-related eruption can also occur after starting topiramate or after a dose change, but timing alone cannot confirm the cause.

The clinician will ask when the rash began, whether it is spreading, which medicines and products you use, and whether you have fever, mouth sores, eye symptoms, facial swelling, or skin pain. Do not diagnose it from photographs or assume every rash is a drug allergy.

Contact the prescriber promptly about a new rash and give the medicine name, dose, start date, recent changes, and a clear description of the skin. Do not restart, stop, or change topiramate without medical guidance unless emergency clinicians tell you to do so; sudden changes may affect the condition it was prescribed to treat. Avoid adding new creams or fragranced products before the rash is assessed, and take photographs to show how it evolves.

Seek emergency help for trouble breathing, throat or tongue swelling, faintness, widespread blistering, skin peeling, severe skin pain, purple or target-like spots, fever with a rapidly spreading rash, or sores in the mouth, eyes, or genital area. These features can signal a serious reaction and should not wait for a routine appointment. A mild, localised itchy rash still needs prompt prescriber advice.
