Topical silicone gel for keloid scars: what to know

A keloid is raised scar tissue that extends beyond the original injury and may continue to thicken, itch, feel tender, or cause tightness. Silicone gel forms a flexible layer over intact skin. It is generally used only after the wound surface has closed, because applying products to an open wound can irritate it or interfere with care.

A skin lump is not automatically a keloid. Acne marks, ordinary scars, cysts, and other growths can look different and need different management. A clinician can examine the border, texture, colour, symptoms, and history of injury or piercing.

Clean and dry the healed area, apply a thin layer as directed, and allow it to dry before covering or dressing the site. Stop and seek advice if the gel causes persistent itching, rash, blistering, or worsening redness. Keep the product away from eyes and mucous membranes, and follow the label or clinician’s instructions rather than increasing application.

If the scar keeps enlarging or symptoms interfere with sleep, movement, or clothing, arrange a dermatology review. Options may include steroid injection into the skin, laser treatment, cryotherapy, or surgery, often selected according to the scar’s location and behaviour.

Arrange prompt assessment for rapid enlargement, bleeding, ulceration, marked pain, warmth, pus, or a new lump that has not been diagnosed. Seek urgent help for severe swelling with breathing or swallowing difficulty, particularly after a procedure near the neck or face.

Ask whether the scar is a keloid, when silicone can start, how long to continue, and whether combination treatment would better control thickness, itch, or colour.
