Too Much Pain During Sex: What to Know

Pain may be at the vaginal opening, deeper in the pelvis, or both. Burning or stinging can occur with vulval skin conditions, vulvodynia, irritation, or low lubrication. Deep pain can be associated with endometriosis, an ovarian endometrioma, pelvic inflammatory disease, or pelvic muscle guarding. Menopause-related tissue changes can also cause dryness and soreness. Pain during sex is a symptom with several possible causes, not a diagnosis by itself.

The timing matters: pain with entry, thrusting, or afterward can point clinicians toward different examinations. A clinician may ask about bleeding, discharge, itching, urinary symptoms, periods, contraception, pregnancy possibility, and prior procedures. These details help select appropriate tests and treatment.

Stop or change sexual activity when pain begins, and explain what feels uncomfortable to your partner. Allow more arousal, use a plain lubricant if dryness contributes, and avoid fragranced washes or products on the vulva. Arrange a medical appointment if pain recurs, affects intimacy, or leads you to avoid sex. Do not self-treat suspected infection without an assessment.

Seek urgent medical advice for severe or rapidly worsening pelvic pain, fever, vomiting, faintness, heavy bleeding, new pain during pregnancy, or pain with unusual discharge. Arrange prompt sexual-health or gynaecology care after possible exposure to a sexually transmitted infection, especially with pelvic pain or fever. A sudden severe pain episode needs immediate assessment.

Ask whether the pain is superficial or deep, which examination or swabs may help, and whether pelvic floor assessment is suitable. Ask how to manage dryness or skin irritation safely, whether your symptoms fit endometriosis or another pelvic condition, and when sex can be resumed comfortably.
