Tonsillitis without antibiotics: what to know

Tonsillitis has more than one possible cause. Antibiotics work against certain bacteria, not viruses, so they may add side effects without helping when the illness is viral. Symptoms such as cough, runny nose, or hoarseness can fit a viral pattern, while marked throat inflammation, tender neck glands, fever, and no cough may prompt a clinician to consider strep throat. Appearance alone cannot settle the question.

A clinician may assess your throat, glands, hydration, and general condition before recommending observation, testing, or treatment. Do not use someone else’s prescription or stop a prescribed course without checking with the prescriber.

Drink often, rest, and choose cool or soft foods if swallowing hurts. Gargling with warm salty water may soothe some adults and older children who can gargle safely. Use paracetamol or another suitable medicine only according to the label and your personal medical advice. Avoid smoking and keep separate cups and utensils while unwell. If symptoms are not settling, arrange a GP consultation.

Get medical advice if pain prevents normal drinking, symptoms worsen, or there is fever, a rash, swollen glands, or repeated tonsillitis. Seek urgent care for breathing difficulty, drooling, inability to swallow fluids, a very muffled voice, severe one-sided swelling, or inability to open the mouth. A child who is unusually drowsy or passing very little urine also needs prompt assessment.

Ask what cause is most likely from the examination, whether testing is needed, how long to monitor symptoms, which pain medicine is suitable, and what change should trigger a review.
