Short answer

A toenail may become thick after injury because damage to the nail growth area changes how the new nail is formed.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common trigger
A blow, crush, or repeated pressure
Helpful care
Roomy footwear and gentle trimming
Do not do
Do not pull off attached nail

What the change can mean

A heavy object, tight footwear, repeated running pressure, or a blow can disturb the nail and the skin beneath it. The nail may look ridged, raised, split, or partly detached as it grows forward. A bruise under the nail can also make the nail seem darker while it moves out with growth.

Injury-related thickening does not by itself identify the cause. Nail fungus can also cause a thick, crumbly nail, and nail psoriasis may create ridges, lifting, or colour change. A clinician can examine the nail and, when needed, test material from it before choosing treatment.

What you can do now

Keep the toe clean and dry, trim only loose or sharp edges, and use footwear with enough room for the nail. Do not pull off an attached nail or dig underneath it. Protect a tender toe from further knocks while the nail grows out. Take a clear photograph occasionally so changes in colour, shape, or separation are easier to describe.

Avoid starting antifungal medicine solely because the nail is thick after trauma. If examination suggests fungus, treatment options may include [Oral Antifungal Treatment](/treatments/medical-dermatology/oral-antifungal-therapy/) or other approaches such as [Laser Treatment For Nail Fungus](/treatments/laser-and-energy/laser-nail-fungus-treatment/).

When to seek care

Arrange an assessment if pain is worsening, the toe is red or swollen, drainage develops, or the nail is increasingly loose. Seek prompt care after a major crush injury, especially if walking is difficult or the toe looks misshapen. A dark mark that does not move outward with nail growth also deserves examination.

If thickening persists without a clear injury, or several nails change, ask about [Nail Fungus](/conditions/nail-fungus/) and [Nail Psoriasis](/conditions/nail-psoriasis/) rather than treating by appearance alone.

Questions for your doctor

Ask whether the growth pattern fits trauma, whether a fungal test is useful, and how to trim or protect the nail while it grows. Also ask which changes should prompt an earlier review.

Questions people ask

Will a thick nail after injury return to normal?

It may improve as new nail grows, although the shape can remain uneven if the growth area was significantly disturbed.

Does a thick toenail always mean fungus?

No. Trauma can thicken a nail, but a clinician may test for fungus when the appearance or course is uncertain.
