TNM staging for lung cancer: what it means

TNM staging describes the lung tumour, nearby lymph nodes, and whether cancer has spread elsewhere. Your team combines these findings into a stage that helps compare the extent of disease and plan care.

T describes the size and local reach of the main tumour, including nearby structures it may involve. N records whether regional lymph nodes contain cancer and which node areas are affected. M indicates whether cancer has travelled to distant organs, such as the brain or another part of the body. Imaging, examination, bronchoscopy, biopsy and sometimes surgery can contribute to the assessment. A stage is an organised summary, not a prediction of one person’s experience.

The final stage can change when more tissue or scan information becomes available. Lung cancer type, tumour features and your general health also influence treatment choices. Genomic tumour profiling may identify features that affect which medicines are considered.

Ask for the T, N and M categories in writing and ask which overall stage they represent. Confirm whether the assessment is based on scans alone or includes biopsy and lymph-node sampling. Discuss whether surgery such as lung lobectomy, radiotherapy or chemotherapy is being considered, and how each option fits the findings. If cancer has spread, ask how treatment goals and supportive care will be addressed. Take a medicines list and a written record of symptoms to appointments.

Contact your cancer team promptly for new or worsening breathlessness, chest pain, coughing blood, confusion, a new severe headache, weakness on one side, or difficulty speaking. A hoarse voice can have many causes, but a persistent change should be assessed. Seek emergency help for severe breathing difficulty, sudden neurological changes or heavy bleeding.
