What should you know about tilted uterus pain during sex?

A uterus that angles backwards is often a normal variation. During deep penetration, certain positions may place pressure on the cervix or surrounding pelvic structures. Pain can be deeper, position-specific and worse around menstruation, but the pattern alone cannot identify the cause.

A clinician may consider endometriosis, an ovarian endometrioma, pelvic inflammatory disease, genitourinary syndrome of menopause, lichen sclerosus or vulvodynia. Pain during sex is a common presentation of several pelvic and vulval conditions, so an examination may be needed.

Stop or change any position that causes pain, use slower penetration and consider a lubricant if dryness contributes. Note whether pain is at the entrance or deeper, when it occurs in your cycle, and whether bleeding, discharge, itching or pelvic pain accompanies it.

Arrange a gynaecology assessment if pain persists, returns often or affects intimacy. Treatment depends on the cause and may include pelvic-floor care, treatment for dryness or infection, or a procedure such as laparoscopic excision when endometriosis is confirmed.

Seek prompt medical care for pain with fever, unusual discharge, bleeding after sex, vomiting, faintness, a new severe one-sided pain, or possible pregnancy. Sudden severe pain needs urgent assessment.
