Thunderclap headache after orgasm: what to know

Sexual activity can raise heart rate and blood pressure, and some people develop a headache during arousal or at orgasm. A gradual, familiar headache may have a less serious explanation, but pain that explodes to maximum intensity is different. Its timing after orgasm does not identify the cause.

Serious problems involving blood vessels around the brain can also begin with a sudden severe headache. You cannot reliably separate these possibilities by pain quality alone, and feeling better later does not remove the need for assessment.

Stop sexual activity and sit or lie somewhere safe. Call emergency services or attend an emergency department immediately, particularly if this is the first episode or the pain peaked within moments. Record when it started, how quickly it became severe, and whether vomiting, neck pain, weakness, confusion, fainting, or vision changes followed.

Do not drive yourself if you feel unwell or have neurological symptoms. Avoid further exertion until a clinician has assessed the episode, and bring your medicines list, including any blood thinner.

Emergency help is needed for any first sudden severe headache during or after orgasm. It is especially urgent with fainting, seizure, weakness, numbness, speech difficulty, vision loss, repeated vomiting, fever with a stiff neck, or recent head injury.

Seek urgent assessment even if the pain has eased. A clinician may need to perform an examination and arrange tests before calling the headache safe to manage as a recurring exertional pattern.

Ask: Could this be an exertional or sexual-activity headache? What tests are needed to exclude a serious cause? When can activity resume? What should I do if the pain returns, and could any medicine increase my risk?
