Throbbing cluster headache: symptoms and next steps

Cluster headache commonly causes intense pain on one side, around or behind the eye or temple. The same side may develop a watery or red eye, blocked or runny nostril, facial sweating, or a drooping eyelid. Attacks can start suddenly and may make a person restless rather than wanting to lie still. Episodes may recur in a pattern, while the pain-free intervals can be much longer. A new severe headache still needs assessment because other conditions can mimic it.

Record the start time, duration, location, eye or nose symptoms, possible triggers, and any medicines taken. Rest in a quiet space, avoid alcohol if it has triggered attacks before, and do not exceed label directions for pain medicines. Clinicians may discuss treatments designed for rapid relief and prevention rather than relying only on ordinary painkillers.

Seek emergency help for a sudden explosive headache, new weakness or numbness, confusion, seizure, fainting, fever with a stiff neck, vision loss, or headache after a significant injury. Get prompt assessment for a first severe one-sided headache, a major change in your usual pattern, or pain with a red painful eye. Do not drive yourself if vision or alertness is affected.

Ask whether the pattern fits cluster headache or another cause, which acute treatment is suitable, how to prevent repeated attacks, and which symptoms require emergency care. Bring a diary covering timing, side, associated eye or nose changes, sleep, alcohol, and medicines.
