Understanding a hoarse voice

The vocal cords sit in the voice box and vibrate to make sound. Swelling, dryness, overuse, infection, or irritation can make the voice rough, weak, breathy, or lower than usual. A cold, chronic cough, or acid reflux may affect the throat and voice. Less common causes include thyroid conditions or a problem affecting the throat, lung, or voice box; hoarseness alone cannot identify one of these causes.

Tell a clinician whether the change began suddenly, follows heavy voice use, comes with pain or swallowing difficulty, or is associated with a neck lump, breathing change, or unexplained weight change. Smoking history and occupational voice use also matter.

Protecting your voice

Drink water regularly, avoid shouting and whispering, and take voice breaks. Avoid smoke and other inhaled irritants. Treating a blocked nose or reflux may help when those problems are contributing, but persistent hoarseness should not be self-treated indefinitely. Do not start antibiotics or steroid medicines without assessment.

When to seek care

Get urgent help for breathing difficulty, inability to swallow saliva, rapidly increasing neck or throat swelling, or a sudden voice change with other neurological symptoms. Arrange a medical review for hoarseness that persists, recurs without a clear trigger, or occurs with pain, swallowing trouble, coughing blood, a neck lump, or unexplained weight loss. A clinician may examine the throat and decide whether specialist assessment is needed.
