Thigh pain during sex

A position that stretches the inner thigh, a strained muscle, or involuntary tightening of the pelvic floor can cause pain during penetration or afterward. Pain may also be referred from the vulva, vagina, cervix, ovaries, or pelvis. Endometriosis, an ovarian endometrioma, pelvic inflammatory disease, genitourinary syndrome of menopause, lichen sclerosus, or vulvodynia may present with pain during sex. The underlying condition presents with pain; the symptom does not establish a diagnosis.

Describe whether the pain is at the opening or deep inside, whether it occurs with certain positions, and whether it is burning, aching, sharp, or cramping. Dryness, itching, discharge, bleeding, urinary symptoms, or period-related pain can help guide assessment.

Stop or change activity when pain begins. Try positions that reduce depth or inner-thigh stretch, take more time for arousal, and use a suitable lubricant if dryness contributes. Do not apply fragranced products to the vulva. Gentle hip and thigh movement may help muscular discomfort, but avoid stretching an acutely painful area.

Book a gynaecology review if pain recurs, affects intimacy, or continues after sex. The clinician may examine the vulva and pelvis and consider tests based on your symptoms. Treatment is directed at the cause; laparoscopic excision of endometriosis may be discussed only when endometriosis is diagnosed and appropriate.

Seek urgent help for severe or sudden pelvic pain, faintness, fever, vomiting, heavy bleeding, or pain with possible pregnancy. Arrange prompt care for new bleeding after sex, foul-smelling discharge, sores, marked swelling, or pain that is worsening. These signs can indicate infection or another problem needing timely treatment.

Ask whether the pain seems muscular, vulval, vaginal, or pelvic; whether you need an examination or scan; and how to protect comfort during sex. Mention cycle timing, dryness, discharge, bleeding, and previous pelvic conditions.
