What should you know about TFCC central disc tear?

Short answer

A TFCC central disc tear is damage to the cushioning centre of the wrist’s triangular fibrocartilage complex, often causing pain with rotation or gripping.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the tear means

The TFCC sits between the ends of the forearm bones and the small bones of the wrist on the little-finger side. Its central disc helps spread load. A central tear can follow a fall, forceful twisting, repeated loading, or gradual tissue wear. People may notice ulnar-sided wrist pain, clicking, reduced grip, or discomfort when turning a key or opening a jar. Wrist pain can have several causes, so an examination is needed to distinguish a TFCC problem from a fracture or another tendon or joint condition.

What you can do next

Arrange an assessment if symptoms persist or interfere with work, sport, sleep, or everyday tasks. A clinician may check tenderness, grip, wrist movement and forearm rotation, then decide whether imaging is useful. Early care often reduces aggravating lifting and twisting, uses a supportive splint for a limited period, and gradually restores movement. Physiotherapy can guide strengthening and load management. In selected cases, a cortisone injection may be discussed; it is not suitable for every wrist problem. Do not force painful rotation or return abruptly to heavy gripping.

When to seek care

Seek prompt medical care after a significant injury, visible deformity, marked swelling, or inability to move the wrist or fingers. Urgent assessment is also sensible if the hand becomes numb, unusually pale or blue, cold, or weak. Arrange a routine review for ongoing little-finger-side pain, repeated clicking, or symptoms that are worsening despite rest, because persistent symptoms may need a more specific diagnosis.

Questions for your doctor

Ask whether the findings fit a central TFCC tear, which activities should be limited, whether a splint is appropriate, and how to progress exercises. You can also ask what imaging would clarify the diagnosis and when an injection or surgical opinion would be considered.

Questions people ask

Can a central TFCC tear heal without surgery?

Some tears improve with activity changes, support and guided rehabilitation, but the plan depends on the tear, stability and symptoms.

Why does turning the forearm hurt?

Rotation loads the TFCC and can irritate the injured tissue, particularly when combined with gripping.
