Terrible heartburn in pregnancy: what to do

The valve that normally keeps stomach contents down can relax during pregnancy. Pressure in the abdomen can also push acid upward. Burning in the chest, regurgitation, sour fluid, belching and irritation in the throat are typical reflux features. Gastritis, a peptic ulcer or a hiatus hernia can produce overlapping upper-abdominal symptoms. The sensation should not automatically be labelled heartburn if it is severe, unfamiliar or linked with exertion.

Have modest portions more often, chew well and stay upright after meals. Keep a note of foods and drinks that bring symptoms on, and avoid lying flat soon after eating. A clinician can advise on a safe sleeping position and suitable medicine. Check every over-the-counter product with a pharmacist, including antacids, because timing can affect supplements and prescribed medicines.

Call emergency services for pressure-like chest pain, pain spreading to the arm, jaw or back, severe breathlessness, collapse, blue lips or heavy sweating. Seek urgent assessment for blood in vomit, black stools, severe upper-abdominal pain, repeated vomiting or inability to swallow. Book a clinical review when burning keeps returning, disrupts sleep or continues despite the plan given to you.
