What terminal ileal disease means

The terminal ileum is the last part of the small intestine, where the bowel joins the large intestine. Crohn's disease here can cause cramping around the lower right abdomen, loose stools, tiredness or reduced appetite. Inflammation may also cause the bowel wall to swell or form scar tissue that narrows the passage. A diagnosis is assessed using your symptoms, examination, blood tests, stool tests and imaging or an endoscopic examination when appropriate. Abdominal pain and diarrhoea can have many causes, so symptoms alone do not establish Crohn's disease.

How treatment is planned

A clinician may use anti-inflammatory or immune-targeting medicines to calm active disease and reduce the chance of further inflammation. The choice depends on severity, previous treatment, other health conditions and whether infection, an abscess or a narrowing is present. A dietitian can help if eating is difficult, weight is falling or particular foods worsen symptoms; avoid making major restriction changes without guidance. Keep a record of pain, bowel movements, fever, weight change and medicines. Follow-up may include a GP Consultation, Blood Test and bowel imaging or endoscopy arranged by the treating team. Surgery may be considered for a persistent obstruction, abscess or bowel damage that does not respond to medical care.

When to seek care promptly

Seek urgent medical help for severe or worsening abdominal pain, repeated vomiting, a swollen abdomen, inability to pass stool or gas, heavy rectal bleeding, faintness or a high fever. These symptoms can signal obstruction, bleeding or infection. Arrange a clinical review soon if diarrhoea persists, you are losing weight, eating becomes difficult or treatment is causing new problems. Do not stop prescribed Crohn's medicines suddenly without speaking with your care team.
