Terminal hydrosalpinx: what it means

The fallopian tubes connect the womb with the ovaries. In a hydrosalpinx, inflammation or scarring blocks the tube and fluid collects inside it. Terminal means the blockage is toward the end nearest the ovary. Some people have no symptoms and discover it during fertility assessment; others notice pelvic discomfort, unusual discharge, or pain that comes and goes.

Past pelvic infection, including Pelvic Inflammatory Disease, is one possible contributor, but a scan finding does not by itself prove the cause. Ultrasound and sometimes additional imaging help clarify the tube’s appearance. Your clinician will interpret the result alongside symptoms and examination findings.

Arrange a gynaecology review to discuss the report, symptoms, infection assessment, and whether the other tube appears healthy. Tell the team if you are trying to conceive, considering fertility treatment, or have had previous ectopic pregnancy or pelvic surgery. Treatment is individualised rather than based on the word terminal alone.

If the tube is affecting fertility treatment or causing repeated problems, discussion may include Salpingectomy, which is surgery to remove a fallopian tube, or another procedure selected by a specialist. IVF may be considered when the tubes are blocked or when other fertility factors are present. Ask about recovery, alternatives, and effects on future fertility.

Seek urgent medical help for severe one-sided pelvic pain, fainting, shoulder-tip pain, fever, vomiting, or heavy bleeding, especially if pregnancy is possible. These symptoms need prompt assessment because pelvic infection and ectopic pregnancy can become serious. Contact your clinician sooner if pelvic pain or discharge is new or worsening.

Ask which tube is affected, how confident the imaging diagnosis is, whether infection testing is needed, and how the finding changes your fertility plan. Clarify the reason for any proposed surgery or IVF, expected recovery, and what symptoms should trigger urgent help.
